Refinery Fire Investigation Case Study

Problem

Our client experienced a major fire caused by a rupture of a pipeline and could not detect the cause of the rupture. Because the refinery had a sister unit with the same pipe design, the client was concerned that an additional failure may occur.

Solution

KHE performed pipe stress analysis models on site and formulated results within two days. We determined that the failure of an inadequate support system caused the problem and thus made recommendations to improve the sister unit design.

Result

KHE's Emergency Response Team saved the client valuable time and prevented further shutdown by isolating the cause and offering the best solutions in a new design.
